Cannabis use is increasing throughout the United States, including for women during pregnancy and postpartum. Outcome data regarding cannabis use during pregnancy are limited due to confounding variables and poor methodology. Data thus far highlight an association with preterm birth, low birth weight, and long-term neuropsychiatric adverse outcomes in offspring. Providers should discuss cannabis use with patients, identify reasons for use, treat co-occurring substance or psychiatric disorders, and discuss known and unknown risks of cannabis use to mother and baby. Further research into outcomes related to perinatal cannabis use, treatment of cannabis use disorder, and public health initiatives is necessary.
